ADHD Assessment Checklist

The steps listed below are necessary to ensure accurate diagnoses and avoid untimely delays.

The process of diagnosing a patient with Adult ADHD is a multi session process. If these forms are not completed or you would prefer to do them in-person, then this information will be gathered during your initial appointment. As such, this may delay the commencement of any prescribed medication to your third appointment.

After your first appointment.

Step 1: Online Forms

Please ensure the following secure online forms are completed:

1. The Initial ADHD questionaire.

This takes approximately 45 minutes but is very important to safely and legally ascertaina diagnosis of ADHD.

2. The ADHD Informant questionaire.

Please send this secure online form to two people.

One person should be someone who has known you from childhood AND the other should be a person who has known you as an adult.

In order to ascertain a diagnosis of ADHD two informants are required. One informant should provide history regarding your childhood symptoms. This person should have known you since childhood eg. parents, sibling, aunt/uncle. The second informant should provide history regarding your adult symptoms eg. partner, parents, siblings, aunt/uncle.

Please note: in order to skip sections of the informant forms that are not relevant (eg. if you are only commenting on the Adult ADHD symptoms and not the Child ADHD symptoms), then please just tick “no, none of the examples were applicable” for each section.

Step 2: Routine Tests

Please ensure the you book an appointment with your GP and completed the following tests listed below. A referral form should have been provided to you during your 1st appointment. Unfortunately, given safety and legal parameters, ADHD medication cannot be commenced till these tests are received. It may take up to 1 week for the tests results to be finalised, so please attend to this as soon as possible.

Physical examination with your GP.

  • A letter will have already been sent to your GP with a request for this physical examination. In case this has not been received, please request your GP to complete a a basic cardiac and respiratory examination to rule out signs of underlying cardiac disease that would suggest certain ADHD medications are contraindcated. ​Please ask your GP to have the results of this physical examination sent to info@neuraliatms.com.au
  • If possible, please take a photo of the GP letter describing the physical examination and email it to info@neuraliatms.com.au. GP clinics are often very busy and may not have time to fax the letter.

Routine blood tests ​

  • A number of medical conditions can influence the decision for what medication you may safely have. For this reason you have been provided with a routine blood test form to be completed at your nearest pathology centre.

Urine Drug Screen

  • It is a requirement of the WA Health Medicines and Poison Regulation Branch that you undergo urine drug screening before you commence a stimulant. Further urine drug testing will be requested during your treatment to ensure ongoing compliance with state regulations. Please have your Urine Drug Screen completed at your nearest pathology centre. The Urine Drug screen is included in the blood test form. Please call the pathology clinic before you turn up and confirm, as not all clinics do urine drug screens.

ECG

  • A number of cardiac conditions can influence which ADHD medications you may have. For this reason you have been provide with a ECG form/ Please attend your local Cardiology testing centre (eg. Western Cardiology, Global Cardiology). An appointment is not usually required. It can take up to 1 week for results to be finalised. Occasionally a cardiologist may be required to determine your suitability for certain medications.
